Short Hours Form Now Live!

Beginning May 1, 2019, you are able to find the Short Hours form online on the Registrar's website under Quick Links, Forms, Academic Records. Once you click on the form itself, you are requested to log in to begin the submission. The initiator will fill in the student (searching by name or University ID), program of study, student's advisor, and Department DEO/Director/Chair. You will then check which sessions the form is applicable during the academic year (Summer 2019-Spring 2020), move on to enrollment status (definitions below that field), and activities to qualify that enrollment status. We tried to replicate the paper form as much as possible. After the initiator has completed all the fields, you will Submit to Workflow. The initiator will continue to have View Only access.

If you have the need to do a retroactive Short Hours form (Spring 2019 or before), we ask that you submit on paper as that is our office's trigger to do additional reporting to external agencies.

If you submit a paper form for the semesters that are on the Workflow form, our office will be returning those to you to submit via Workflow. May 1st will be our date for going only on Workflow for Summer 2019 and forward. Paper forms that have already been submitted have been processed.
